A major grant is changing the way healthcare is delivered at health centers in two Northern Michigan communities.
The department oversees Pellston's Hornet Health Center and the Ironmen Health Center in Mancelona.
Part of the grant will help integrate mental health care and physical health care at Pellston's Hornet Health Center, which serves children and young adults 5-21 years old.
A portion of the grant will also help develop a Pathways Community HUB model at the Ironmen Health Center in Mancelona, which serves young people ages 10-21. The pathways model helps get resources to the community members.
